It was Wyong's third consecutive grand final loss.
The Roos took a 12-6 lead into halftime, but could not hold off the Tigers who dominated the second stanza.
Wyong football manager Stephen Marks said The Entrance opened the scoring in the 17th minute.
"Fullback Grant Nelson received the ball wide on the right from a block play, burned his marker on the outside, and raced around the in-goal to improve the position for his skipper Matt Killick who converted," Marks said.
It took Wyong until the 31st minute to open their account.
"The Tigers failed to secure a late tackle kick by Wyong five-eighth Luke Sharpe, and surrendered field position to the Roos," Marks said.
"Wyong probed the defence with a series of forward charges, before interchange forward Jack Lote repeated the exercise, pivoted, and carried several defenders over the line to score."
Wyong's Josh Mantellato converted the try to equalise at 6-all.
Marks said Wyong was "most fortunate" to score again right on the halftime break.
"A scoreboard malfunction forced the timekeeper to manually sound the halftime siren as the Roos pressured the Tigers' line," he said.
"Referee Jeff Hartup did not hear the original siren.
"Sharpe grubbered towards the in-goal. The Entrance spilled the ball as Hartup acknowledged the second siren. Sharpe recovered the ball and dived over from close range.
"Mantellato converted to give his team an unlikely 12-6 lead going into the break."
Mantellato extended Wyong's lead to 14-6 when he booted a penalty goal in the 50th minute.
Wyong lost possession from the restart but defended the ensuing five tackles.
"The Tigers' principal kicker, Will Pearsall, was corralled in possession and attempted to kick the ball while being held by three defenders," Marks said.
"The ball rebounded off the legs of one of those defenders."
The Entrance was awarded six more tackles, although many in the crowd doubted that Wyong had played at the ball.
From the ensuing tackles, The Entrance shifted the ball right and Jacob McCudden stepped inside his marker and surged over to score.
Killick converted for a 14-12 scoreline with more than 20 minutes to play.
"The match had entered the championship rounds," Marks said.
"Wyong made two clean breaks via centre Luke Dolbel and half Terence Seuseu but was thwarted by desperate, scrambling defence.
"The Tigers elected to run the ball on the final tackle in the 73rd minute and shaped to the right.
"As the Wyong defence advanced to meet the threat, a pass from Shaune Corrigan was knocked forward by a Wyong hand straight into the arms of Tigers' Matt Nelson.
"The right winger seized the ball at pace and had too much momentum for the Roos' defence."
His try shot The Entrance to a 16-14 lead.
Nelson's attempt to convert his try was waved away.
Marks said Wyong simply didn't have sufficient petrol in the tank to run in a try in the closing minutes.
"The Roos appeared spent and the Tigers were largely untroubled over the final few minutes to secure the title."
Killick was named Player of the Match.
