The 78-year-old was last seen leaving his home about midday on Sunday, May 12.
About 12.10pm on Tuesday, the man was located by rescue crews partly submerged in water in a culvert along the side of the Pacific Highway, Doyalson.
The man was removed from the culvert and treated for hypothermia, dehydration, cuts and bruising.
He was taken to Wyong Hospital in a stable condition.
Police thanked the media and the public for their assistance.

Officers from Tuggerah Lakes Police District will today (Tuesday) continue their search to locate a 78-year-old man who went missing from his home at Doyalson at the weekend.
Ernest Mackay was last seen at his Scenic Highway home about midday on Sunday. He left the home soon after and hasn't been seen since.
Family of Mr Mackay reported his disappearance to police due to concerns he had wondered off into bushland that surrounds his home. Police were also told Mr Mackay was in the early stages of dementia and may have become disorientated.
He is described as Caucasian, about 175cm tall with a thin build and grey/black hair. He was last seen wearing King Gee overalls and white gum boots.
Police searched from Scenic Drive at Doyalson until 12.30am on Tuesday, and will resumed the search at 9am today.
Local police will be assisted by personnel from RFS, SES and the VRA.
