Rain forced the postponement of the Saturday fixture for 24 hours, and the change in scheduling meant the Hammerheads went into the game without several key players.
Southern Lakes trailed 24-14 at the break, but piled on 20 unanswered points in a dominant second-half display.
The club has now sealed the A grade minor premiership and club championship.
